Licensed Practical Nurse (LPN)

About Us: Join ACI Federal's dynamic healthcare team! We are currently seeking a skilled and compassionate Licensed Practical Nurse (LPN) to contribute to the delivery of exceptional long-term care.

Position Overview: As an LPN, you will play a vital role in providing high-quality care. Your responsibilities will include:

Assisting new residents in adjusting to the unit, philosophy, and goals.
Reporting any accidents or changes in the resident's condition to the Charge Nurse and documenting on progress notes in the chart.
Collaborating with the interdisciplinary team to prepare, implement, and evaluate care plans.
Discussing clinical issues regarding resident care with the Charge Nurse.
Observing, recording, and reporting progress or changes in the resident's condition.
Updating care plans as needed.
Administering and dispensing medications (excluding IV) to residents, observing for side effects and effectiveness.
Securing and accounting for controlled drugs.
Carrying out therapeutic treatments.
Maintaining nursing care and administrative records and reports.
Attending staff meetings and participating in in-service programs.

Minimum Qualifications:

Completion of a course of study approved by the New York State Education Department as qualifying for Licensed Practical Nurse.
Licensure to practice as a Licensed Practical Nurse in New York State.
COVID Vaccination required.
