The primary focus of this position involves performing the mandated service of long-term support services (*LTSS) to conduct pre-admission screening and medical need determination for Medicaid-funded nursing home placement and community-based services (PAS/NHS). This position serves as a collaborative partner working with the Department of Social Services (DSS) to conduct joint home visits to assess/screen individuals for Medicaid-funded nursing home placement and other referral services; pediatric LTSS screenings are completed independent of DSS. This requires completing a nursing assessment and completion of the Universal Assessment Instruments (UAI). The UAI is a tool used to determine medical eligibility of vulnerable members of our population for Medicaid funded Long-term Services and Supports (LTSS). The nurse is this position will also provide resources and referrals to at-risk clients and serves as a limited case manager for screening follow up needs. The successful applicant works independently, is well organized, promotes teamwork and communicates effectively with patients, staff, community groups, private providers, *and district management.

Minimum Qualifications:
This position requires licensure as a RN in Virginia and must have current BLS-CPR certification. Must have experience with medication administration, injectables, and specimen collection. The incumbent must exercise sound nursing judgement, have effective verbal, written, interpersonal/physical assessment skills and be able to work with persons from multicultural and diverse backgrounds. Must be able to be fit tested in accordance with OSHA Respiratory Protection Standard (29 CFR 1910.134) [Ref.2] regulations. Must be able to have work experience demonstrating decision making skills, patient teaching, interviewing, counseling as well as be able to analyze situations and problem. Experience in home visiting and case management is needed. Working experience in use of computers and computer applications, including internet, MS Office- Word, Excel, MS Edge, Google and other cloud-based applications. Travel to client homes or approved community sites is required, an occasional weekend and/or evening hour may be required.
Preferred Qualifications:
Knowledge and experience in LTSS screening and/or using E-pas application preferred; current or previous work experience in public health preferred; experience with disease surveillance, TB, epidemiology, communicable diseases, or refugee health guidelines is a plus; English/Spanish bilingualism desired.
