Job Title: Case Manager III/Service Coordinator III

Job Summary:

Support children (birth to age three) with developmental delays and their families by assessing needs, creating service plans, coordinating care, and monitoring progress. Maintain compliance with agency and regulatory documentation requirements.

Responsibilities:

  • Manage early intervention referrals and service coordination.
  • Develop and update Individualized Family Service Plans (IFSPs).
  • Ensure compliance with HIPAA, Medicaid, and other regulations.
  • Conduct regular home and community visits to assess progress.
  • Connect families with resources, services, and entitlements.
  • Monitor service delivery, family satisfaction, and IFSP timelines.
  • Collaborate with service providers and agencies for continuity of care.
  • Educate and support families, including crisis intervention when needed.
  • Maintain records and attend required meetings and training.

Requirements:

  • Knowledge of early intervention, case management, Medicaid, and developmental disabilities.
  • Strong skills in assessment, coordination, advocacy, and crisis management.
  • Ability to work flexible hours, including evenings/weekends as needed.
  • Must be able to lift 40 lbs and provide transportation if necessary.
